Registered Nurses or Registered Nurses provide professional care to patients/significant persons through personal and/or telephone contact in accordance with established standards and practices. He/she coordinates with other members of the care team to ensure seamless care, maximum coordination, and active patient participation in planning and care. Assist physicians or other providers with clinical procedures, conduct telephone screening as needed, and participate in patient education.

- Report to work on time or notify the patient care manager or designee when the employee knows they will be absent or late, and/or in accordance with department protocols

- Enroll in all required CareMount health plans and workplace services

- Accept responsibility for reading, understanding, and adhering to the information contained in the current CareMount Medical Employee Handbook, Procedures Manual, and CareMount Medical Clinical and/or Administrative Policies, as applicable.
- Maintain patient confidentiality in all circumstances and adhere to all CareMount Medical confidentiality policies
- Whenever possible, assist the Patient Care Manager and/or designee and/or other administrative personnel with alternate assignments or other responsibilities, overtime, or changes in normal business hours to ensure the efficient operation of CareMount Medical.

- Demonstrate clinical competence in the direct care of patients/significant persons in an established area of practice, taking into account developmental differences between patients
- Demonstrate basic knowledge of common disease processes and treatments, including relevant diagnostic/laboratory tests and medical/nursing interventions; become familiar with preventive health practices relevant to the population
- Demonstrates a proactive approach to patient care that focuses on meeting the individual and family needs of each patient at the time of service; communicates identified needs in a timely manner
- Using an evidence-based approach when working with patients/families/carers to support self-management, self-efficacy, and health-promoting behavior change
- Works with the patient/partner and other members of the health care team to establish goals and priorities to meet the patient's current and future needs; actively participates in the care team to ensure that the patient's needs and preferences are included in the comprehensive care plan
- Use population-specific knowledge and clinical experience to facilitate ongoing assessment and effective management of the healthcare needs of a specific patient population
- Perform call triage with special emphasis on assessment, problem/priority identification, and decision making
- Demonstrate knowledge, skills and technical competence in the application of medications/therapy; take prescribed medication correctly, understand exact dosage, route, location and possible side effects
- Provide accurate and up-to-date clinical information to patients in accordance with the scope of professional nursing practice and accepted CareMount standards of medical practice, policies and protocols.
- Problem solving/decision making using nursing processes
- Competently perform authorized technical procedures (eg, EKG, phlebotomy, IV initiation/monitoring, selected laboratory tests, etc.) as required by the area of clinical practice.
- Demonstrate clinical competency while assisting a physician or other provider with procedures
- Seek confirmation/guidance from physicians or other providers and nursing colleagues as needed
- Work effectively with other health care providers during emergencies
- Understand and correctly and consistently practice Universal Precautions and follow all applicable OSHA guidelines
- Make sure all clinical equipment is kept clean and in working order.

- Keep lines of communication open with CareMount physicians and other medical staff within and between departments, even during the most stressful periods.
- Maintain open lines of communication with physicians, pharmacies, and other non-CareMount Medical agencies and services in the community
- Notify patient/partner and/or front desk staff immediately of unexpected doctor/other provider delays or schedule changes
- Demonstrate effective communication strategies that reflect the needs of all patients, including those from vulnerable groups
- Reassure patients who are anxious and/or need additional help or information
- Direct and coordinate the work of LPNs, Physician Assistants, and other support personnel to meet identified patient care needs.
- Maintain the confidentiality of all information obtained from patient records and conversations with patients/significant persons, physicians and other CareMount medical personnel
- Answer the phone promptly (within three rings) courteously and introduce yourself by name and department (and at the doctor's office, if necessary, by the doctor's name)
- Be discreet and sensitive in all telephone conversations.
- Use a non-judgmental approach when speaking with patients and colleagues.
- Efficiently and effectively triage patient calls and schedule same-day (in-office) appointments based on individual or departmental medical guidelines
- Forward calls only when necessary according to CareMount Medical policy
- Facilitate patient referrals and precertification as necessary with managed care plans in which CareMount Medical participates; communicate with outside organizations and/or providers about the possibility of additional care coordination when identified
- Provide clinical advice to CareMount Medical patients in accordance with departmental protocols, individual physician or other provider instructions, and/or carer's accepted practice
- Use available computer/email systems for interdepartmental and intradepartmental communication in accordance with agreements between the department and/or CareMount Medical
Patient/other important teaching/education:
- Use clinical knowledge and skills to effectively teach patients/significant persons (eg, through written information, face-to-face communication, telephone communication)
- Incorporate patient and family education (including health/wellness promotion) into care delivery
- Continuously evaluate the results of patient education and modify educational programs based on patient results.
- Advise and/or facilitate referral (for example, social services, nutritionist, hospice) (if applicable)
- Design and implement educational activities that are culturally appropriate (and consistent with individual medical practice) and tailored to the patient's/other person's ability to understand and participate
- As requested, participate in the development of patient education tools/programsdesignated department

- Receive full messages for all calls, including, but not limited to: correctly spelled full name and current phone number of the caller, date, time, and reason for the call; sign all messages with your full name
- Document all relevant clinical observations, medications/vaccines/treatments, allergies, etc. in the EHR.
- Keep clear records or documentation of all calls received, including but not limited to: date and approximate time of the call, full name, date of birth (if applicable), phone number of the caller, and reason for the call
- Document relevant phone calls and significant nurse-patient interactions in the patient's EHR, including but not limited to: nursing advice or information given to the patient/carer/significant person (on behalf of the patient), medical advice or information given to the patient as directed physicians or other service providers, communication with pharmacies, managed care companies, etc.
- Record the full name of the physician or other provider on the EHR when recording information or advice as directed by the physician or other provider
- Sign all medical record entries in accordance with CareMount Medical policy
